This is just a simple CUBES math strategy poster. Each letter is on a single 8.5" x 11" page. I have this poster as part of my math focus wall along with posters with common keywords, vocabulary, and habits of mind.
This is a student friendly writing rubric that also comes in a half-page size. I also include a grading slip that only has the name of the trait and a box to write the grade for the trait. These make for a quick way to assess and attach to a writing project for feedback.
This is a good resource if you are unable to pay for data processor. This is an Excel Spreadsheet with 10 separate spreadsheets in one file. It was made to work with up to 3 classes to track grade-level progress in DIBELS Next measures for 4th Grade. The spreadsheets include One for Beginning of the year (BOY), one for middle of the year (MOY), and one for the end of the year (EOY) and one Class Report for each of the three classes. It also includes a Grade Level Report sheet to track the prgress of the grade level as a whole. Each of the classes has linked pages allowing you to enter the name of a student on the BOY page and it will update the MOY and EOY sheets for that class. Each class report page includes a pie chart for each measure showing the class breakdown for each measure. The grade level report includes the pie charts for the same measures at the grade level. Each sheet will allow you to enter the students' names then enter their scores for the DORF, Accuracy, Retell, and Daze and it will automatically change the background color of the cell to Red, Yellow, Green, or Blue based on the DIBELS Next targets and cut points for FOURTH GRADE (based on the current data of 10/25/2018). It will also calculate the student's composite score and color that as well. This is an outline for use with the Step Up To Writing curriculum. It is intentionally made with no color to allow the students to color the parts of the outline during the lesson.

This is a simple weekly report that I send home on Friday to give parents a snapshot of how their student is doing in class, and if there is any missing work or behaviour problems. It's fully customizable in Excel.
This is a simple, single-page, data table and graph that will show the aim line, progress monitoring, and benchmarks for a student. You enter the beginning benchmark score and the end of the year target on the table and the software will plot this on the graph. You can then enter up to 4 progress monitoring points per month and these will also be plotted on the graph. You can also enter the Benchmark scores for these to be plotted. It makes a easy to read data page with the visual representation of the graph. It is nice for RTi meetings as well as conferences. This is an Excel spreadsheet and is fully customizable. It has separate pages for 20 students. Because you enter your own targets and data it can be used for virtually any progress monitoring or data tracking over time.
